DUTIES:
Evidence based measures
* Oversees the collection, compilation, analysis and presentation of evidence based measure data as assigned; * Prepares and presents requested reports regarding the findings and outcomes of evidence based Measures; * Reviews concurrently those patients that may trigger assigned evidence based measure care paths; * Cross trains with other coordinators to be able to assure department function continuity in the event of illness or other staff absence; * Understands all elements of the Comet database and is proficient in use; * Serves as the Administrator for the Comet database, as assigned; * Serves as the Administrator for the QNet Exchange website, as assigned; * Ensures reliability and validity of the evidence based data and able to maintain 90% proficiency in validation scores; * Facilitates timely submission of Comet data; * Assists Manager and/or Director in data gathering and analysis as requested; * Supports and collaborates with Chief Medical Officer and other physician leadership as directed for presentation of outcome data at department meetings for the purpose of quality improvement; * Serves as a resource to peers, colleagues, management and medical staff regarding requirements of evidence based measures, as assigned.
Regulatory Standards
* Maintains a current working knowledge relative to state and Federal standards and regulatory requirements (i.e., CMS Conditions of Participation, Joint Commission standards, state hospital licensure, etc.) * Maintains a current working knowledge of relative patient safety standards and national patient safety initiatives and outcome measures (i.e., Leapfrog, IHI, evidence based measures, etc.); * Participates in facility tracers to ensure ongoing compliance, document and evaluate performance improvement requirements for Joint Commission, CMS and other applicable compliance requirements; * Participates effectively in performance improvement initiatives such as FMECA, RCA, and Sentinel event investigations and related corrective action planning; * Works collaboratively through daily rounds with hospital wide departments to support ongoing compliance with Federal, State and regulatory statutes and standards, and medical staff and hospital staff education Skills Requirements * Makes suggestions for improving Hospital compliance.
Department Support
* Readily communicates all opportunities to improve processes, policy, and procedures; * Supports and promotes the mission of the hospital and the Quality Department in providing safe, effective patient care; * Maintains a team centered attitude within the department with peers and management staff.
Medical Staff Support
* Works collaboratively with Medical Staff coordinator to ensure ongoing medical staff physician profiling activities are completed timely and within the defined facility structure for credentialing activities; * Facilitates peer review committee meetings as assigned * Takes and produces accurate minutes for peer review committees as assigned and according to hospital policy; and
Other duties as assigned.
Qualifications
Skills Requirements:
* Demonstrates ability to effectively communicate within all levels of the organization; * Demonstrates effective leadership skills in advancing patient outcomes (i.e., through evidence based measures, performance improvement reviews and teams, etc.); * Demonstrates knowledge of data systems and computer software resulting in ability to effectively and efficiently create accurate data sets used for reporting, analysis and presentation; * Detail oriented to ensure accuracy of work product and promote credibility within all levels of the organization; * Ability to demonstrate current knowledge of clinical care guidelines and national priorities for safe, effective patient care; * Knowledge of medical terminology and disease processes sufficient to provide accurate, efficient medical record reviews and interpretation of care standards; * Analytical skills sufficient to accurately and efficiently evaluate outcomes of care and ability to lead others in the processes involved in a team environment; * Effectively communicate results of patient care evaluations sufficient to meet the objectives of improving patient care; * Leadership as a role model in communicating the goals and objectives of the Quality Department with the medical staff, administration, management and all hospital staff in order to further the Hospital's performance improvement initiatives and activities.
Qualifications:
* RN Licensure * Minimum of 3 years experience in an acute care setting. (OR or ICU experience is a plus). * Knowledge of computer software. * Proven ability to work effectively with hospital and medical staff. * Understanding of research principles and scientific methods. * Skilled in accurate, efficient medical record review. * Sufficient knowledge to interpret care standards. * Flexibility in scheduling. * Highly motivated and organized with the ability to focus on project development. * The employee will be proficient in performing the functions of this job after six months of orientation.
